Apply for Your PWD ID

Information we collect

Personal details (name, date of birth, sex, civil status), disability information (type and cause), residence address, contact details (mobile number, landline, email), educational attainment, employment and occupation, organization affiliation, ID reference numbers, family background (father, mother, guardian), the certifying physician’s name and license number, and the files you upload (medical certificate, valid ID, 1×1 photo and optional signature).

How we use your information

• To check and verify your application and supporting documents.
• To process, approve and issue your PWD ID.
• To keep and update the PWD records of the municipality.
• To contact you (by email or mobile number) about your application and to send your login credentials.

Who has access

Your information and documents are accessible only to authorized MSWDO Peñaranda personnel through their official accounts in this system. They are not sold and are not used for any purpose unrelated to the PWD ID program. They are disclosed to other government offices only when required by law or by the PWD ID / registry program.

How long we keep it

Your information is kept only for as long as needed to process your application, maintain PWD records, and comply with government record-keeping rules.

Your rights

Under the Data Privacy Act of 2012 you have the right to be informed, to access and correct your information, to object, to withdraw your consent, to request blocking or removal where allowed by law, and to file a complaint with the National Privacy Commission (privacy.gov.ph). Withdrawing your consent may mean your application can no longer be processed.
